CONTRACT FOR PERSONAL SERVICES
Addendum made this 20th day of June, 2012, between the City of Ashland ("City")
Siegel Planning Service LLC ("Consultant").
Recitals:
A. On March 1, 2012 City and Consultant entered into a "City of Ashland Contract for
Personal Services" (further referred to in this addendum as "the agreement').
B. The parties desire to amend the agreement to extend the date of completion to
August 31, 2012.
City and Consultant agree to amend the agreement in the following manner:
1. The date for completion as specified in Paragraph 3 of the agreement is extended to
August 31, 2012.
2. Except as modified above the terms of the agreement shall remain in full force and
effect.

Unified Land Use Ordinance 67,460.00
Project will be completed in two
phases. PHASE I is $30,000, will be
completed by June 30, 2012 and will
involve preparation of the code outline
and first draft of the Unified Land Use
Ordinance (see Tasks 1-3 in attached
statement of work).
PHASE II is$34,720.00, will be
completed by June 30, 2013, and will
involve the incorporation of graphics,
green building and economic development
best practices evaluation and code
language, and a final revised unified
land use ordinance (see tasks 4-7 in
the statement of work).
Note: Phase II is subject to budget
approval.
Contract for Phase I is attached.
