Alsing NAME Dept. of Public Works Date May 26, 19 71 Cit H 11 ADDRES shTana Oregon 97520 Phone 482-3211 This permit is granted to you subject to the terms and conditions stated below, and on reverse side, and not otherwise; and the violation of any of said terms or conditions will constitute sufficient cause for cancellation of this permit. ANY WORK STARTED ON THE CONSTRUCTION OF ANY PORTION OF THE PIPELINE OR UTILITY INSTALLATION DESCRIBED BELOW SHALL CONSTITUTE YOUR ACCEPTANCE OF THE PROVISIONS OF THIS PERMIT. Description of installation -_InOtall 4" Lateral Sewer Line from 855 CLAY STREET and a V4" Water Service to cross CLAY STREET to connect with main water line. Location of installation See attached sketch. (Road, Section, Township & Range TERMS AND CONDITIONS OF THIS PERMIT: 1. This permit is granted for use only on roads under the jurisdiction of the Jackson County Court and is not valid upon any federal highway, state highway, city or town street, or any road not on the county road system of Jackson County. 2. Proper barricades and warning signs must be maintained at all times during construction by the holder of this permit so as to insure the safety of the motoring public. 3. Backfill over any portion of the pipeline or utility installation described above that lies within a constructed road- bed shall be thoroughly tamped to prevent settlement. The top 12 inches of backfill shall be crushed gravel. Balance of backfill• shall be as stated in special conditions below. 4. Jackson County assumes no liability for any damage which may be caused to the pipeline or utility crossing described above due to routine road maintenance or road improvement. It shall be the responsibility of the holder of this permit to provide for the construction of the pipeline or utility described above in such a manner as to have suf- ficient depth and length so as not to interfere with normal road maintenance operations. 5. The holder of this permit shall hold the County of Jackson harmless and blameless from any damages that may be caused by the installation or maintenance of the above-described pipeline or utility crossing. 6. Special conditions All excavations on shoulders to be backfilled as per Item #3 above and excavations within roadway to be backfilled entirely with 3/4-0 rock and asphalt replacement to be with hot mix asphalt. Please read provisions listed on reverse side. This permit is revocable at any time, and will be strictly adhered to and no work other than that specifically men- tioned above is hereby authorized. This permit shall be void unless the work herein contemplated shall have been completed before 'y---2.6-............ 19--7-0----. Earth backfill over pipe shall not exceed six (6) inches in depth. Unless otherwise specified in special conditions. 2. All excess rock or dirt to be removed from traveled portion of the road by brooming, or washing as directed. 3. Final cleanup to be completed within a reasonable period. •.J - 3- 4. Cable or pipe shall be covered at a depth of thirty (30) inches minimum. 5. Any pavement cut (asphalt pavement to be sawed) will be replaced to depth of three (3) inches with hot-mix asphalt within three (3) working:days_from the date of completing trench opening. 6. No litter, debris, boulders, or waste will be permitted to accumulate. 7. The Jackson County Engineer's Office is to be notified in writing, or by telephone (773-6211 - Ext. 251 or 252) three (3) days prior to starting of work. 8. No less than r/2 of the roadshall be in good repair and open to the traveling public at all times. Unless adequate detour is provided. 9. All work areas to be replaced to an "as good as, or better than" condition as existed prior to this install- ation. Holder of this permit to be responsible for any trench settlement for two years following install- ation. 10.
