ADDENDUM TO CITY OF ASHLAND
CONTRACT FOR PERSONAL SERVICES
Addendum made this 2nd day of December, 2015, between the City of Ashland ("City")
and URS Energy & Construction Services. Inc. ("Consultant").
Recitals:
A. On March 15. 2015 City and Consultant entered into a "City of Ashland Contract for
Personal Services (further referred to in this addendum as "the agreement").
B. The parties desire to amend the agreement to extend the date of completion,
increase the compensation to be paid Consultant and modify the proiect.
City and Consultant agree to amend the agreement in the following manner:
1. The date for completion as specified in Paragraph 3 of the agreement is extended to
June 30, 2016, the maximum price as specified in Paragraph 4 of the agreement is
increased to $57.081.00).

SUBJECT: Reeder Gulch Project No. 1107-OR
Dam Abutment Erodibility Evaluation and Geologic Update
Request for Increase in Authorization
Please refer to our February 12, 2015 Proposal for Services - Hosler Dam Left
Abutment Erodibility (Revised). The final deliverable from the scope of work described
in that proposal, titled Dam Abutment Erodibility Evaluation and Geologic Update (Rev
1), was published October 30, 2015.
The FERC issued a letter dated November 6, 2015 in which it was discussed to defer
the plan and schedule until after a face to face meeting involving the City, the
Independent Consultant, and the FERC could be held. This has been scheduled for
December 15, 2015. Further, per our telephone conversation on November 18, 1 have
agreed to present our study to the Ashland City Council, scheduled for December 14,
2015. The proposal defining this scope of work included an allowance for telephone
conferences with the FERC, but did not include an allowance for travel or labor to
develop presentation materials for in-person meetings. This request for increase in
authorization covers these added costs and also includes review time for John France,
of our Denver office, who has expertise in Risk-informed Decision Making (RIDM), which
potentially may offer an attractive approach to mitigation of the identified
erosion vulnerability.
Our itemized costs are summarized in Table 1.

contract which was fully executed on May 14, 2015. Charges will be billed on a time and
materials basis not to exceed the contract ceiling, as modified by this request and in
accordance with our present contract.
