Project Background OBEC understands that the City of Ashland will be applying for funding to design and construct the East Nevada Street Extension Project consisting of a new two-lane bridge over Bear Creek in order to better connect neighborhoods along Mountain Avenue with the north end of Ashland. The City of Ashland is seeking funding application support for two scenarios. Scenario 1: Feasibility and associated costs of constructing one bridge with bike ped facilities combined, either to the middle or to one side of the bridge. Scenario 2: Feasibility and associated costs of constructing two separate bridges, one for vehicles and one for bike ped only. OBEC has prepared the following proposed scope of services for the project. Scope of Work Task 1 - Project Management and Coordination The major objectives of this task are to schedule, coordinate and supervise Project work, and to establish lines of communication between OBEC and City of Ashland staff. OBEC shall prepare and provide monthly project invoices with progress reports. For budgeting purposes, it is assumed that two (2) OBEC staff members will attend two (2) public review meetings for up to three (3) hours (including travel) to discuss the Project. Scott Fleury Cotvsaum January 19, 2016 m 1 ENGAUEEBS Page 2 INE&I lw~ Task 2 - Project Investigation, Concept Layouts, and Scoping The major objectives of this task are to utilize previously gathered information from Nevada Street Bridge scoping (OBEC Job #P019-0412), consider opportunities and constraints for two bridges, and determine the constraints for each concept for the different scenarios. In addition, OBEC will utilize data already gathered to provide an exhibit for each scenario to illustrate the transition of bike/ ped facilities from the road to each proposed scenario. Task 3 - Environmental Assessment Based on the results of Task 2, OBEC will investigate and document environmental concerns associated with permitting for each scenario. This analysis will be completed from computer desktop research and no site visit will be necessary. The results will be presented in a brief technical memorandum that clearly lists the expected permits and environmental clearances for each scenario including expected timeline for agency review and approval and any associated fees. Estimate to complete environmental clearances and permits will be included in the Cost Estimate under Task 4. Task 4 - Cost Estimating Based on the results of Task 2 and 3, OBEC will prepare detailed scoping estimates for both scenarios of the project in spreadsheet format. The estimates will include all major project elements, including design and construction engineering, environmental clearances, bridge and roadway construction items, temporary items like traffic control and erosion control, and contingencies. Calculations and assumptions will be provided to allow for easier review. Fee OBEC proposes to complete the project on a time-and-materials basis with a not-to-exceed limit of $6,576. A breakdown of projected hours for each task is attached in Exhibit A. Project billings will be prepared on a monthly basis based per the attached 2015 OBEC rate sheet. We appreciate the opportunity to present this proposal to you for this project. Please do not hesitate to contact me if you have any questions or need additional information. A person or organization is an additional insured under the provision only for that period of time required by the contract, agreement or permit. With respect to the insurance afforded to the additional insured, this insurance does not apply to: "Bodily injury", "property damage" or "personal and advertising injury" arising out of the rendering of, or failure to render, any professional architectural, engineering or surveying services, including: (a) The preparing, approving, or failure to prepare or approve, maps, shop drawings, opinions, reports, surveys, field orders, change orders, designs or drawings and specification: or (b) Supervisory, inspection, architectural or engineering activities. The limits of insurance that apply to additional insureds are described in Section D. Limits Of Insurance. How this insurance applies when other insurance is available to an additional insured is described in the Other Insurance Condition in Section E. Liability And Medical Expenses General Conditions. No person or organization is an insured with respect to the conduct of any current or past partnership, joint venture or limited liability company that is not shown as a Named Insured in the Declarations. Other Insurance If other valid and collectible insurance is available for a loss we cover under this Coverage Part, our obligations are limited as follows: When You Add Others As An Additional Insured To This Insurance: That is other insurance available to an additional insured. However, the following provisions apply to other insurance available to any person or organization who is an additional insured under this Coverage Part: (a) Primary Insurance When Required By Contract: This insurance is primary if you have agreed in a written contract, written agreement or permit that this insurance be primary. If other insurance is also primary, we will share with all that other insurance by the method described in c. below. (b) Primary And Non-Contributory To Other Insurance When Required By Contract: If you have agreed in a written contract, written agreement or permit that this insurance is primary and non-contributory with the additional insured's own insurance, this insurance is primary and we will not seek contribution from that other insurance. Rev 5.14 Page 1 of 2 BUSINESS LIABILITY COVERAGE FORM Summary SS 00 08 04 05 Paragraphs (a) and (b) do not apply to other insurance to which the additional insured has been added as an additional insured. c. Method Of Sharing If all the other insurance permits contribution by equal shares, we will follow this method also. Under this approach, each insurer contributes equal amounts until it has paid its applicable limit of insurance or none of the loss remains, whichever comes first. If any of the other insurance does not permit contribution by equal shares, we will contribute by limits. Under this method, each insurer's share is based on the ratio of its applicable limit of insurance to the total applicable limits of insurance of all insurers. Waiver of Subrogation If you have waived any rights of recovery against any person or organization for all or part of any payment, including Supplementary Payments, we have made under this Coverage Part, we also waive that right, provided you waived your rights of recovery against such person or organization in a contract, agreement or permit that was executed prior to the injury or damage Rev 5.14 Page 2 of 2 Client#: 331197 OBECCONE FDATE (MMIDD/YYYY) ACORDTM CERTIFICATE OF LIABILITY INSURANCE 1/21/2016 THIS CERTIFICATE IS ISSUED AS A MATTER OF INFORMATION ONLY AND CONFERS NO RIGHTS UPON THE CERTIFICATE HOLDER.
