For the Ashland Senior Program's Services in providing information and assistance services to Jackson County clients 60 and
older (III B) for Senior and Disability Services for Fiscal Year 2010-2011 in accordance with the contract between the Rogue Valley
Council of Governments (RVCOG) and the Ashland Senior Program. This contract runs from July 1, 2010, until June 30, 2011, or the
date that RVCOG accepts the Ashland Senior Program's completed performance, whichever date occurs first.
Certificate Holder CANCELLATION: Should any of the coverage documents herein be cancelled before the expiration date
thereof, CIS will provide 30 days written notice to the certificate holder named herein, but failure to mail
Rogue Valley Council of Governments, such notice shall impose no obligation or liability of any kind upon CIS, its agents or representatives, or the
