Description:
For the Ashland Senior Program's Services in providing Older Americans Act Title IIIB information and assistance services to seniors
age 60 and older for Fiscal Year 2013 in accordance with the contract between the Rogue Valley Council of Governments and the Ashland
Senior Program through the City of Ashland Parks and Recreation Department. This contract runs from July 1, 2012, until June 30, 2013, or
the date that RVCOG accepts the Ashland Senior Program's completed performance, whichever date occurs first. RVCOG and its divisions,
officers, and employees are Additional Members but only with respect to the Ashland Senior Program's services to be provided under said
contract.
Certificate Holder: CANCELLATION: Should any of the coverage documents herein be cancelled before the expiration date
Senior & Disability Services thereof, CIS will provide 30 days written notice to the certificate holder named herein, but failure to mail
Rogue Valley Council of Governments such notice shall impose no obligation or liability of any kind upon CIS, its agents or representatives, or
and its divisions, officers, and employees the issuer of this certificate.
