ADDENDUM TO CITY OF ASHLAND
CONTRACT FOR PERSONAL SERVICES LESS THAN $35,000
Addendum made this 27th day of June, 2018, between the City of Ashland ("City")
Construction Engineering Consultants, Inc. ("Consultant').
Recitals:
A. On November 20, 2015, City and Consultant entered into a "City of Ashland Contract
for Personal Services Less than $35,000" (further referred to in this addendum as "the
agreement').
B. The parties desire to amend the agreement to increase the compensation to be paid
to the Consultant and extend the date of completion.
City and Consultant agree to amend the agreement in the following manner:
1. The maximum price of the agreement is increased by 2 000, creating a new
overall purchase order total of $10,515.
2. The date for completion of the agreement is extended to June 30, 2019.
3. Except as modified above the terms of the agreement shall remain in full force
and effect.

Per your request, we have estimated the additional engineering work required for the
project due to changes in the ODOT-PROWAG requirements as listed below.
Total anticipated cost of additional services $2,000.00
