GOODS & SERVICES AGREEMENT
between
THE CITY OF ASHLAND,
an Oregon municipal corporation,
and
a (nonprofit), Southern Oregon
Forest Restoration Collaborative
("Contractor")
for
Services for deliverables to provide a monitoring summary for Ashland Forest Resiliency
Stewardship Project (AFR).

RECITALS
A. The City and Provider previously entered into a Personal Services Agreement effective
September 1, 2025 (the "Agreement"), for monitoring summary of AFR and deliverables for an
Oregon Watershed Enhancement Board grant,
B. The City and Provider now wish to amend the Agreement to extend the term the Agreement to
reflect the intent of the original proposal.
AGREEMENT
NOW THEREFORE, in consideration of the mutual benefits and obligations set forth
herein,the parties agree as follows:
1. Amend the Term of the Agreement in the Scope of Work that set the performance period of
9/l/2025 to 6/30/2026 as outline on page I of 8 on the Personal Services Agreement between
City of Ashland and Southern Oregon Forest Restoration Collaborative. The new term of
agreement deadline will be amended to September 30, 2026, to reflect the extended
performance period of the Oregon Watershed Enhancement Board grant extended timeline.
2. This Amendment No. I is effective upon being signed by both parties.
3. Except as specifically modified by this Amendment No. 1,the terms and conditions of the
Agreement remain in full force and effect.
