As the owner(s) of the property listed above, I hereby consent to the following improvements, dedication, or other
actions as Fequired by the City of Ashland, and agree to bear the propo,~ionate paymentof associated costs. This Agreement
is to be binding upon myself/ourselves, my/our heir(s), executors, and assigns, and it is my/our express intention that this
Agreement shall run with the land, so that fulfillment of the items listed below shall be binding upon future owners of the
property.

1 ) THE OWNER HAS RECEIVED A BUILDING PERMIT TO INSTALL A GUEST HOUSE ABOVE THE GARAGE
AND THIS DOES NOT CONSTITUTE APPROVAL OF A SEPARATE RESIDENTIAL UNIT, NOR A
TRAVELERS ACCOMMODATION UNIT. THE OWNER SHALL NOT INSTALL A KITCHEN IN THE GUEST
HOUSE FOR THE PURPOSES OF A SEPARATE, RESIDENTIAL UNIT WITHOUT SITE REVIEWAPPROVAL.
THE OWNER HAS CONDITIONAL USE PERMIT APPROVAL FOR FIVE TRAVELERS ACCOMMODATION
UNITS IN ADDITION TO THE OWNERS RESIDENCE. A SEPARATE, RESIDENTIAL UNIT WITHOUT SITE
REVIEW APPROVAL, OR A TRAVLERS ACCOMODATION UNIT WITHOUT A CONDITIONAL USE PERMIT
IS A VIOLATION OF THE ASHLAND MUNICIPAL CODE AND SUBJECT TO ALL PENALTIES THEREOF.
