COMMUNITY FORESTRY ASSISTANCE GRANT PROGRAM
GRANT AGREEMENT BETWEEN
OREGON DEPARTMENT OF FORESTRY
and
City of Ashland
GRANT NUMBER: CFA97-1
The Oregon Department of Forestry (hereafter referred to as "grantor" ancllor
"Department") agrees to provide City of Ashland (hereafter referred to as "recipient")
the sum of $1.150 for the expressed purpose of conducting a community forestry
project using funds provided through the U.S. Forest Service.
The grant administrator for the grantor is Paul D. Ries, Urban and Community
Forestry Program Manager, Oregon Department of Forestry, 2600 State Street,
Salem, OR, 97310 (503) 945-7391. Recipient agrees to appoint and maintain a
designated project coordinator for the duration of the project.
A copy of the proposed project submitted by recipient is hereby considered to be a
part of this agreement, subject to the modifications made below under Section 1.
SECTION 1: TERMS AND CONDITIONS
1.1. Projects may begin upon execution of this agreement and must be
completed by March 1, 1998, unless otherwise authorized by grantor.
Interim deadlines may be established by the Department in the event of a
lack of progress on the part of the recipient.
1.2. Project coordinator will provide progress reports, in writing, to the grant
administrator. Any significant changes to the grant project must be
submitted in writing to and approved by the Department. A completed final
report form will be provided with the final reimbursement request.
1.3. Modifications to project as directed by DOF: The grant review committee
suggested that your budget line item for supplies could be acquired from
other sources, so your grant award is $400 lower than requested.
Additionally, it is the policy of the Department of Forestry to approve of all

species planted through grant projects.
SECTION 2: MATCHING FUNDS AND REIMBURSEMENTS
2.1. The recipient's financial contribution, as detailed in the proposal, will be
provided. Grant amount will be equally matched with tax revenues,
donations, or in kind contributions as specified in proposal. No portion of the
matching funds submitted by the recipient will be derived from federal
monies.
2.2. The CFA Grant Program is a reimburseable grant program. Grant recipients
must complete or make significant progress on the proposed project before
grant funds will be dispersed. Grant funds are made on a reimburseable
basis, after the recipient has encumbered grant related expenses.
Recipients may request disbursement of grant in no more than two
installments. Matching funds expended by the recipient must be equal to or
exceed the grant installment amount requested unless approved by the
Grant Administrator.
2.3. A form will be provided to the recipient for recording expenditures of both
grant funds and the local match. The Department shall disperse grant funds
within 30 days of receiving a properly completed invoice voucher from
recipient.
SECTION 3: CERTIFICATIONS
3.1. Recipient agrees to allow the Oregon Department of Forestry the right to
periodic inspection of the proposed project.
3.2. Any product, publication, and or publicity must list the Oregon Department
of Forestry and the USDA Forest Service as sponsors and/or financial
contributors to the project. The Department will receive, from the recipient,
two (2) copies of any publication or report arising from the use of these grant
funds.
3.3. Recipient will permit the Oregon Department of Forestry to audit the project
in compliance with the requirements of the Federal Government Single Audit
Act and OMS Circular A-128.
3.4. Upon completion of project, recipient will submit to grantor, a final report and
detailed accounting of budget expenditures for recipient supplied match and
for all grant funds. Report to be filed with grantor no later than June 1, 1998.

3.5. Acceptance of grant implies recognition of the obligation to comply with all
applicable state and federal laws.
4. LIABILITY AND NON-PERFORMANCE
4.1 The recipient agrees to defend or cause to be defended and to indemnify
and hold harmless the state of Oregon, the Oregon Board of Forestry, the
State Forester, and the Oregon Department of Forestry, its officers, agents,
employees, and members, against any and all claims, suits, liens, damages,
or causes of action for damages of any nature resulting from or arising out
of the activities of the recipient, its contractors, subcontractors, agents, or
employees under this agreement.
4.2 The Oregon Department of Forestry reserves the right to withdraw this grant
award for non-performance of grant activities, and recipient agrees to return
any grant funds to the Department in the event the terms, conditions, or
certifications of this agreement are not met to the satisfaction of the Oregon
Department of Forestry and the USDA Forest Service.
