Location of installation
TERMS AND CONDITIONS OF THIS PERMIT:
1. This permit is granted for use only on roads under the' jurisdiction of the Ja<:kson County Bo.ard of Commis-
sioners and is not valid upon any federal highway, state highway, c'ity or town street, or any road not on the county road
system ~f JiJckson County.
2. Proper barricades and warning signs must be maintained at all times during construction by the holder of this
permit so as to insure the safety of the motoring public.

3. Backfill over any portion of the pipeline or utilit~ install~tio~ described above that lies within a ,constructed road-
bed shall be thoroughly tamped to prevent settlement. the top 12 Inches of backfill shall be crushed gravel. Balance of
backfill shall be as stated in special conditions below. .
4. .Jackson Cbunty,assumes no Iiabitity for and damage which may be caused to the pipeline or utility crossing de-
scribed above due to routine road maintenance or road improvement. It shall be the responsibility of the holder of this
permit to provide for the construction of the pipeline or utility described above in such a manner as to have sufficient depth
and length so as not to interfere with normal road mailntenance operations.
5. The holder of this permit shall hold the County of Jackson harmless and blameless from any damages that may be
caused by the installation or maintenance of the above-described pipeline or utility crossing.
6. Special conditions Compliance with the attached four pages of permit conditions.

This permit is revocable at any time, and will be stri,ctly adhered to and no work other than that specifically mentioned

TRENCH BACKFILL MATERIAL - The moisture content of materials
to be mechanically compacted shall be within Plus 2 or Minus 4
percentage points of optimum moisture at the time of compaction.

MINIMUM LENGTH OF OPEN TRENCH - Unless otherwise approved by
the Director, backfilling of
any trench shall be accomplished so that no more than 200 feet
of trench is left open at any time. Cleanup shall be completed
immediately behind the backfilling operation.
BACKFILL MAINTENANCE - The surface of the backfilled
trench shall be watered and
graded as often as necessary to keep the traveled way smooth
and dust free. If required by the Public Works Director, an
approved dust palliative shall be applied.
COMPACTION TESTING - The owner and/or the Public
Works Department will do
compaction testing as soon as practicable after the backfilling.
The Contractor shall provide for opening and rebackfilling the
trench for testing at the locations specified. Whenever a
failing test occurs, additional tests shall be taken to isolate
the section having insufficient compaction. The Contractor
shall immediately recompact the failing section and provide
for retesting as stated above. The compaction requirements
shall be considered as minimum requirements and any future
settling of the backfill shall be considered the result of
insufficient compactive effort. The Contractor shall immediately
repair any sunken area to the satisfaction of the Public Works
Director.
PAVEMENT REPLACEMENT - Immediately prior to patching
the trench with asphaltic
concrete the existing pavement shall be neatly cut with a
pavement saw or other approved breaker. All cracked or ravelled
pavement shall be removed without creating abrupt jogs in the
cut line. Pavement trimming, finishing of the gravel surface,
tacking the edges and pavement replacement shall be performed
only under the immediate supervision. of the Public Works
Director or his authorized representative. Unless otherwise
approved by the Director, all trenches within a paved, traveled
way 'shall be resurfaced with asphaltic concrete within 10
calendar days after testing and approval of the backfill. The
finished surface of the new pavement, when tested with a 10-foot
straightedge, shall not vary from the testing edge by more than
1/4-inch at any point. The top course of asphalt shall be
constructed only by workmen thoroughly familiar with asphalt
finishing work.

TEMPORARY PAVEMENT REPLACEMENT - Whenever weather conditions
prohibit the use of hot mix
asphaltic concrete or if the cOntractor chooses to wait until
all trenching and backfilling is completed, a temporary asphaltic
covering (cold mix or similar) may be constructed. The temporary
surfacing shall be removed in its entirety before placement of
the permanent pavement.
LIGHTING, SIGNING AND DETOURS -
1. Flagmen shall be required whenever the Contractor's
operations are inclose proximity to the travel portion
of the roadway. Sufficient flagmen, pilot cars and
signing shall be provided to minimize the hazard to
traffic while the work is in progress.
2. Reflectorized advance signing and lighted barricades
shall be placed to adequately delineate open trenches,
berms, equipment or any other hazardous condition in
the roadway at night. The amount and placement of such
signing and barricades shall be no less than that specified
by the County Public Works Director.
3. All requests for detours and lane closures shall be
submitted with three copies of the detour plan showing
traffic signing proposed. No detour or lane closure will
be permitted until approval of the plan by the County
Public Works Director. When the plan is approved the
Contractor shall notify all emergency agencies, school
districts and postal carriers concerning the location
and duration of the detour.
CLEANUP AND REPAIR - All areas affected by the
construction shall be brought
to an "as good as or better" condition prior to completion of
the project. Repairs shall include, but are not limited to:
1. Pavement Repairs - In addition to reconstruction
within trench area, the"
Contractor shall repair any other pavement, base material
or subgrade damaged as a result of his operations. This
includes damage to shoulders and pavement edges caused by
detouring traffic and equipment around the work area. In
case of excessive damage, an asphaltic overlay will be
required.
2. Contaminated Gravel Surface - All gravel surfaces contaminated
with mud, dirt, oversize rock
or other foreign material shall be removed .and replaced with
3/4"-0 crushed rock meeting the requirements hereinabove set
forth.

3. Slopes and Roadside Ditches - Shall be trimmed, smoothed
and compacted to the original
lines and grades.
4. Driveways, Culverts and Ditches - Shall be replaced to the
original lines and grades.
5.
General Cleanup -
The Contractor shall at all
