As owner of the property listed above, I hereby consent to the following improvements, dedication, or other actions as
required by the City of Ashland, and agree to bear the proportionate payment of associated costs. This Agreement is
to be binding upon myself/ourselves, my/our heir(s), executors, and assigns, and it is my/our express intention that
this Agreement shall run with the land, so that fulfillment of the items listed below shall be binding upon future owners
of the property.
Action: The owner shall not install kitchen facilities into the lower floor family room. The existing property is
approved only for one single family residence and the lower floor family room will not be utilized as a
separate residential unit. A separate, second residential unit is not permitted without obtaining a
conditional use permit approval, and therefore use of the space as a separate dwelling would be a
violation of t~shland Municipal Code and subject to all penalties thereof.

EXHIBIT "A" to QUICLAIM DEED
Real property at 385 Orchard Street, Ashland, Oregon, situated in the County
of Jackson, State of Oregon, and more particularly described as follows:
Lot Six (6) of SKYCREST HILLS, PHASE 2, In the City of Ashland In Jackson
County, Oregon, according to the OffIcial Plat thereof, now of record.
(Code 5-01, Account #1-091733-2, Map #391 EOSDC, Tax Lot #3011)
SUBJECT TO:
1. Public Utility easement affecting the Southeasterly 10 feet, the
Northeasterly 10 feet and a strip of land, 10 feet in width, lying within a
Northwesterly portion as shown on the recorded plat and declaration.
2. Covenants, conditions, restrictions, easementis, and/or setbacks
imposed by instrument, subject to the terms and prOvisions thereof,
recorded October 13, 1998, as No. 98-47466, of the Official Records of
Jackson County, Oregon.
NOTE: This exception omits any covenant, condition or restriction based on
race, color, religion, sex, handicap, familial status or national origin as
