Survey. In cooperation with the Office of Revenue Sharing, we annually submit the information
obtained through this survey to the Revenue Sharing Certifying Official for certification that all
actions reported were made in accordance with State law, and to ascertain that our records are

The Revenue Sharing Certifying Official has indicated to us that the State is unable to certify
one or more of the boundary change actions which your municipality has reported to us. Please
review this information, which has been listed on the reverse side of this letter. If the items
listed are legitimate boundary change actions, please send a copy of the effecting authorization
(ordinance, resolution, etc.) and a plat to the Revenue Sharing Certifying Official. requesting
that the Revenue Sharing Certifying Official notify the Bureau upon receipt. Should your review
indicate either that the State's records are in error or that information previously reported to
the Bureau regarding boundary change actions needs to be modified, both the Revenue Sharing
Certifying Official and the Bureau should be informed accordingly.
It is important that you notify us of your findings and the actions being taken as soon as possible
so that qualifying changes will receive consideration for Revenue Sharing purposes. If you have
any questions regarding our request, please contact Mr. Robert Bakondi of my staff at

This tract of land was retraced through the utilization of
information which was gathered in the course of surveying that
tract of land shown as Recorded Survey No. 8607, a Major Land
Partition for Mr. and Mrs. Leslie George, which is yet to be
appI'oved by the Jackson County Planning Commission, and this
survey for Lucy W. Harrell. The retracement of the outer
boundaries of the Bellevue Tracts and a legal subdivision of
Section 15 were needed was well as a retracement of the existing
Talent Irrigation District Canal, in order to postion the tract
of land as shown on the accompany map.
Basis of Bearing was derived from the National Oceanic and
Atmospheric Administration (formerly Coast and Geodetic Survey)

January 3, 1984 approved the second reading of the ordin-
ance annexing your property to the City of Ashland, subject
to the payment of the required annexation fee of 5<;: per
square foot.
Upon receipt of your check for $5,094.34, the Mayor
and City Recorder are prepared to sign the ordinance and
send it to the Secretary of State's office in Salem. Upon
notification of acceptance by the Secretary of State, or
thirty (30) days after second reading, whichever occurs
later, the annexation will be effective.

PUBLIC HEARING DATE ON THE lJITHDRAWAL OF CERTAIN REAL PROPERTY
FROM JACKSON COUNTY FIRE DISTRICT NO.5; DIRECTING PUBLICATION
OF NOTICES AS DIRECTED BY ORS 222.52-4: AND DECLARING AN
EMERGENCY. (KREBS ANNEXATION)
THE PEOPLE OF THE CITY OF ASHLAND DO ORDAIN AS FOLLOWS:
SECTION 1. The City of Ashltind finds that the owners of the prOD-
erty described in Exhibit "An attached hereto and by t;his reference
incorporated herein have consented to the annexation of said land
to the City of Ashland.
SECTION 2. The submitting of the question of the said annexation
is hereby dispensed with and a Public Hearing is hereby called at
the Council Chambers, Civic Center, 1175 East Main Street, Ashland,
Oregon, on the 20th day of December, 1983, at the hour of 7:30 P.M.,
at \vhich time all persons may appear and be heard on 1~he proposed
annexation.
SECTION 3. The City Staff is directed to cause notice of said hear-
ing to be given as required by ORS 222.120 so that public notices
are published in a newspaper of general circulation appearing in the
City in issues of such newspaper on December 6Lh and Deceplber 13th ,:'..983,
and that notices be posted in four (4) public places in the City for
a period of not less than two (2) weeks prior to the date of the pubJj.c

SECTION 4. Pursuant to ORS 222.524 a Public Hearing shall be held
on December 20,1983at 7:30 P.M. in the Council Chambers, Civic Center,
1175 E. Main Street, Ashland, Oregon, on the question of the withdrawal
of certain real property from Jackson County Fire District No.5.
All persons may appear before the Ashland City Council and be heard on
the question. The real property above mentioned is situated in Jackson
County, Oregon and more particularly described in Exhibit "A" attached
hereto and by this reference made a part hereof.
SECTION 5. The City Staff is directed to publish and post notices of
said hearing in the same manner and on the same dates: as described on
Exhibit "A" attached hereto relative to the.I'Toposed annexation.
SECTION 6. It is hereby adjudged and declared that the existing con-
ditions are such that this ordinance is necessary for the immediate
preservation of the public health and safety of the people of the City
of Ashland and an emergency is hereby declared to exist and this
ordinance shall take effect and be in full force and effect from and
after its passage and approval by the Mayor.

We hereby request and consent to the annexation of the above-described land to the
City of Ashland; and, in exchange for the sCheduling of a public hearing llnd in consideration
of this request by the City of Ashland, we agree that this consent to annE:xation shall be
non-revokable for a period of three (3) years from its date.
We further agree to pay five cents (5)for each square foot of land sought to be
annexed to the City of Ashland as a fair and joust contribution tOlfard the cost of existing
municipal improvements and utilities that may benefit the land; said payment to be made to
the City of Ashland at the time the land is annexed, or when either water or sewer services
are furnished to the land at our request, whichever Occurs first.
At the time the land described herein is annexed to the City of Ashland, we agree
to pay to the City of Ashland an amount sufficient to enable it to withdraw this land from
all public.service districts as defined by ORS 222.510 and to which it may belong s9 that
this shall be at no present Or future expense to the City of Ashland. With respect to each
such public service -district that, at the time of annexation, has outstanding liabilities
or- indebtedness, we shall pay to the City of Ashland an amOunt equal to the preceeding year's
assessment for such indebtedness multiplied by the number of years remaining on such indebted-
ness.
We agree that this agreement shall be binding upon our heirs and assigns.
