February 16,2010 File No. 232009.012.001 Mr. Scott Fleury City of Ashland 51 Winburn Way Ashland, OR 97520 Reference: Agreement for Professional Services, dated June 22, 2009 Ashland Municipal Airport Runway 12/30 Rehabilitation and PAPI Installation Subject: Supplement No. I, P API Obstruction Location Survey Dear Mr. Fleury: We are pleased to submit the following proposal for the PAPI Obstruction Location Survey at the Ashland Municipal Airport. This letter serves as a supplement to our referenced agreement. The terms and conditions established by the agreement are unchanged except as noted below. A PROJECT UNDERSTANDING Reid Middleton performed a PAPI obstruction survey on July 22,2009, that was part of the original agreement. At that time it was determined that there were obstacles that penetrated the P API obstacle clearance surface at both ends of Runway 12/30, even using the maximum PAPI glide path of four degrees. Due to the nature of the obstacles, removal was not an option, so based on the FAA guideline that no obstruction could penetrate the obstacle clearance surface, the construction of the PAPl's could not be funded. Under the direction of the City of Ashland (City), Reid Middleton prepared and submitted a Form 7460 requesting the use of baffles to remove the obstructions from the P API's field of view, thus making the construction of the PAPl's eligible for funding. Upon reviewing the Form 7460, the FAA determined the P API system could be funded . with the condition that baffles be used to eliminate that portion of the standard. 20 degree viewing angle that is impacted by obstacle penetrations of the obstacle clearance surface. Reid Middleton will perform a survey to gather design data necessary for properly sizing the baffles that are to be used on the new P API systems. Form 7460 for PAPI Approval Coordinate, prepare, and submit Form 7460 to the FAA for PAPI approval. This effort includes completion of the form and a narrative of the proposal with design calculations and exhibits. 2. PAPI Obstruction Location Survey a. Prepare and coordinate with the City, Airport, FAA, and subconsultant for the on-site P API obstruction location survey to be done at the Ashland Municipal Airport. Wicks Surveying will perform this survey (see attached). b. . Coordinate the effort and collect and tabulate results on site during survey. c. Analyze ahd document the results of the on-site survey to be used in the design of the baffles for the P API systems. . C. COMPENSA nON Reid Middleton shall be paid on a time-plus-expense basis using the rates indicated in Exhibit "A," Schedule of Charges Effective July 1,2009, attached to the master agreement. We estimate the fee for this portion of the services to be $15,180. We request the budget for the project be increased $15,180 to reflect this revised scope of services. The amended total for the agreement inclusive of all supplements is increased from $195,399 to $210,579. We appreciate the opportunity to submit this proposed supplemental agreement. The terms of this agreement will become effective when confirmed by your signature within 30 days. If you wish to pursue this project after that time, this agreement may then be renegotiated.
