ADDENDUM TO AGREE!1ENT ,/ BETWEEN THE CITY OF ASHLAND, OREGON AND LOCAL UNION NO. 659 INTER~ATIONAL BROTHERHOOD OF ELECTRICAL WORKERS l,HEREAS, there presently exists an agreement between the parties effective July 1, 1979, through June 30, 1982, covering wages, hours and working conditions, and WHEREAS, the existing Agreement provides for negotiations on employee only dental insurance under Article XX, Part B, effective July 1, 1981, Article XVI, Section I shall be amended as follows: - Page I - . ARTICLE XVI HEALTH, DENTAL, WELFARE AND RETIREMENT Section I. Health, Dental and Welfare: The City agrees to pay the full premium for the duration of this Agreement for the following Health, Dental and Welfare benefits: Group health insurance for the employee and family; Dental insurance for the employee; $10,000 life insurance policy for the employee with AD&D supplements; $1.000 life insurance pOlicy for each of the employees' dependants; salary continuation plan which provides 50% monthly salary to a maximum payment of $1,000 per month; workers compensation insurance as required by law and employer's social security contribution as required by law. WHEREAS, the existing Agreement provides a formula under "Appendix B" which establishes wage rates for July 1 through December 30, 1981. In accordance with that formula, the wage rates for each classification are as follows: - Page 2 - , . It should be understood that while the City is paying the total monthly employee only premium under ODS (Oregon Dental Service) of $8.82, the actual City offer was 15% above the $8.10 cost of Blue Cross Plan No. ~.
