PERSONAL SERVICES CONTRACT
Addendum made this first day of July, 2003, between the City of Ashland ("City") and
Roy Bashaw ("Consultant").
Recitals:
A. On December 20, 2002, City and Consultant entered into a "City of Ashland
Personal Services Contract for Services Less Than $25,000" (further referred to in this
addendum as "the agreement").
B. The parties desire to amend the agreement by changing the completion date
to June 30, 2004.
City and Consultant agree to amend the agreement in the following manner:
1. Paragraph 3 of the agreement is amended by changing the completion date from
June 2003 to June 30, 2004.
