This permit is granted to you subject to the General Conditions stated below and the
Special Conditions attached. Violation of any of said conditions will constitute sufficient
cause for permit cancellation. ANY WORK ON APl\' PORTION OF THE INSTALLATION DESCRIBED BELOW
WITHOUT A VALID PERMIT CONSTITUTES A VIOLATION OF O.R.S. 374.30~.

GENERAL CONDITIONS:
1. See Signing sheet
2. "Caution Buried Power" sign to be installed
1. This permit is granted for use only on roads under' the jurisdiction of the Jackson
County Board of Commissioners and is not valid upon any federal highway, state highway,
city or town street, or any road not on the county road system of Jackson County.
2. ~..~e Jackson County Dei~artment of Public Works is to be notified a minimum of 24 hours
prior to starting work within a C~ounty road right-of-way (776-7268 or 826-3122)..
3. As provided in O.R.S. 758.010 the Ft~blic Works Department, acting on behalf of the
Co,unty Board of Commissioners,· may designate where utilities may be located within a
County road right-of-~.~ay and may 0z-der the lo'cation of such facility changed if deemed
· expedient.
4. Permittee shall be resPonsible fo.r all damages caused by any operatio v; associated
with the utility installation. 'Ail damaged areas shall be restored to an "as good as,
or better than" condition as exist, ed prior to the utility installation.
5. Pez-mittee agrees to hold the County of Jackson harmless and blameless from any damages
that may be caused by the ~nsta~.at~on, maintenance or operation of t}.~-, above-described
utility,
6. Permittee shall be responsible for restoration, and maintenance of all areas adversely
affected by the utiIity installation for a period of two (2) years afte= completion
of the project.
7. By acceptance of'this permit, permittee agrees to be responsible for all permit 'conditions,
including the attached Special Conditions, and said responsibility shall survive the
termination of this permit.
