ASHLAND SENIOR PROGRAMS GONTRACT
For the Period July 1, 1984 through June 30, 1985
The RVGOG Area Agency on Agin9 hereby recognizes and incorporates the
Contractual Services Agreement of December 20, 1984, between the City of
Ashland and ACCESS, Inc.,. as part of the RVCOG Area Agency on
Aging/Ashland Senior Programs contract of July 1, 1984, with the £ollowing
additions and stipulations:
The RVCOG Area Agency on Aging hereby holds the City of Ashland
responsible for all actions and omissions by the subcontractor in
relation to the Contractual Services Agreement.

The eervices subcontracted to ACCESS, Inc., will be limited to
outreach. No other services are recognized or incorporated into
the RVGOG AAA/Ashland Senior Programs contract in this addendum·
Outreach activities will include 500 units of service to 200
unduplicated Ashland area persons aged 60 or over during the
period January 1, 1985 through June 30, 1985.
Payment £or subcontracted outreach services and compliance with
RVCOG AAA reporting requirements remain the responsibility of the
Ashland Senior Programs.
