This permit is granted to you subject to the General Conditions stated below and the
Special Conditions attached. Violation of any of said conditions will constitute sufficient
cause for permit cancellation. ANY WORK ON ANY PORIION 0F IHE !NSIALLATION DESCRIBED BELOW
WITHOUT A VALID PERMIT CONSTITUTES A VIOLATION OF O.R.,S. 374.305.
Description of installation 900 feet of 10" water line
Location of installation TOLMAN CREEK ROAD - from Greenmeadows Way 900' south
GENERAL CONDITIONS:
1. This permit is granted for use only on roads under the jurisdiction of the Jackson
County Board of Commissioners and is not valid upon any federal highway, state highway,
city or town street, or any road not on the county road system of Jackson County.
2. .The Jackson County Department of Public Works is to be notified a minimum of 24 hours
prior to starting work within a County road'right-of-way (776-7268 or 826-3122).
3. As provided in O.R.S. 758.010 the Public Works Department, acting on behalf of the
County Board of Commissioners, may designate where utilities may be located within a
County road right-of-way and may order the location of such facility changed if deeme~
e~edient.
4. Permittee shall be responsible for all damages caused by any operations associated
with the utility installation. Ail damaged areas shall be restored to an "as good as,
or better than" condition as existed prior to the utility installation.
5. Permittee agrees to hold the County of Jackson harmless and blameless from any damages
that may be caused by the insta!!atJon~ ~a~ntenance, or operatien cf the abo~e-desc~ibed
utility.
6. Permittee shall be responsible for restoration and maintenance of all areas adversely
affected by the utility installation for a period of two (2) years after completion
of the project.
By acceptance of this permit, permittee agrees to be responsible for all pe~,it conditions,
including the attached Special Conditions, and said responsibility shall sorviva the
termination of this permit.
