The Oregon Departme:}.t of Environmental Quality wishes to retain your services.
It is understood that you will perform tke following circled tasks:
dSet up and exchange the high volume filters every six (6) days
(from schedule) and mail them to the Department of Environ~rent.%l
Quality Lak. o~atory us{.ng franked envelopes; on occasion, extra
sam?les will be request:ed ~or a short period (e.g., ,~n air st. ag:h~tion
period) .
Check the AISI t. az .... at least c:~ce every six days, noting the tt.~e,
date, spot, and flow rate on the tape or an accompanying leg
Report unusual conditions which ma}, affect or explain an unusual
sample. Examples would be: new construction, fire, equipment
failure, dust st. orm, power outage, etc..

that the contractor is temporarily unable to perform scheduled sampling
and also to notify the Department of Environmental Quality Inorganic
Laboratory at 229-5983, collect.
Samples must be delivered to the Laboratory as soon as possible after
sampling. Samples that are postmarked more than 12 days after s~.mptir~g
will be voided and charged to operator error.
If the Depart~ment requests collection of special samples, they will be
paid at the regular per sample rate.
Payment will be made on a quarterly basis.
It is understood that you will be working as an independent contractor and
will be responsible for any federal_ or state taxes applicable to the services
rendered, and that you will not be eligible for fe~.ar.~l socL~l security,
worker's comf}ensation, unemplo}~nent insurance or Public Employees' Retirement
System benefits as a result of this contract.
The Department certifies that sufficient funds are avai!ab!e to finance
costs of this agreement within its current appropriation or lhnitation
from the current biennimm.
This contract may be terminated by mutual consent of both parties, or by
either party, upon seven days' notice in writing and delivered by certified
mail or in person.
Please indicate your acceptance of the terms of this contract by signing
the original and two copies enclosed and return all copies to the Department
of Environmental Quality, Business Off£ce, 522 S.W. Fifth Avenue, Portland,
Oregon 97204.
You may retain one copy of this letter for your records.
