new curb. Existing pavement to be overlaid with 2" of Class B A.C. Two Type CG-3 inlets or
approved equal to be installed at Sta. 18+37.07 and 19+13.07 on the left side.

terms and provisions contained and attached; and the terms of Oregon Administrative Rule; Chapter 734,
Division 55, which is by this reference made a part of this permit.

3. The work area during any construction or maintenance operation performed under ~h~
permit shall be protected in accordance with current provisions of the "Manual on
Uniform Traffic Control Devices for Streets and Highways, U. S. Department of
Transportation".
4. If a contractor is to perform this work, prior to commencement of any work,
applicant will provide'the contractor an approved copy of this permit and appropriate~,
General Provisions. The contractor shall have this material in his possession while
work is performed.
5. Ail work will be in accordance with Standard Specifications for Highway Construction,
Oregon - 1974.
6. Applicant, or his contractor,'to contact Curt'Yocham, Section Supervisor, at 482-4344
at least 3 days prior to commencement of any work, and 24 hours before all work is
completed or equipment is removed from the work area.
7. Specifications as shown on the permit application take precedence over architect's
and engineers' drawings and must be adhered to.
8. Any new curb cuts are not to be made prior to the abutting property owner obtaining
an approach road permit from the Highway Division.
