Dear Barbara:
U.S. Bank is delighted to extend our banking services contract through June 30, 2005.
This letter will serve as a Notice of Amendment to our contract dated May 2001. The City may
accept this amendment by completing the acknowledgment section below.
We are pleased to offer the City of Ashland the same pricing through June 30, 2005. The
processing fee for Merchant Payment Services will continue at the current as well. Any
Visa/Mastercard price increase is a pass through expense.
U.S. Bank has enjoyed a strong partnership with the City of Ashland for many years. We truly
value this relationship and look forward to supporting your financial objectives in the future.
