CITY OF ASHLAND
CONTRACT FOR PURCHASE OF EQUIPMENT
Contract made this 2nd day of November ,2004, between the City of Ashland
("City") and Altec Industries. Inc. ("Contractor").
City and Contractor agree:
1. Contract Documents: This contract is made as a result of an Invitation to Bid issued
by City entitled "INVITATION TO BID" for an 37' BUCKET TRUCK WITH INSULATED
BOOM ON FORD F550 CHASSIS, Bid No. #103-2004, Contractor was awarded the
in the terms of this contract, the contract documents defined in the invitation to bid and
Contractor's bid, this contract shall take precedence over the contract documents which
shall take precedence over the bid.
2. Scope: Contractor shall produce and deliver the equipment described in the contract
documents within the time prescribed in the contract documents. The following
exceptions, alterations or modifications to the contract documents are incorporated into
this contract:

equipment.

Including the "exceptions/substitutions" as listed on letter dated October 26, 21004
Including the following options:
1. Outrigger over-ride switch as listed $100.00
2. Two (2) Outrigger pads as listed $480.00
3. One (1) deep cycle battery as listed, wired in isolation $260.00
4. Hinged curbside access steps as listed $435.00
