This permit is granted to you subject to the terms and conditions stated below, and on reverse side, and not other-
wise; and the violation of any of said terms or conditions will constitute sufficient cause for cancellation of this permit.
ANY WORK STARTED ON THE CONSTRUCTION OF ANY PORTION OF THE PIPELINE OR UTILITY INSTALLATION DE-
SCRIBED BELOW SHALL CONSTITUTE YOUR ACCEPTANCE OF THE PROVISIONS OF THIS PEBMIT.

TERMS AND CONDITIONS OF THIS PERMIT:
1. This permit is granted for use only on roads under the jurisdiction of the Jackson County Board of Commis-
sioners and is not valid upon any federal highway, state highway, city or town street, or any road not on the county road
system of Jackson County.
2. Proper barricades and warning signs must be maintained at all times during construction by the holder of this
permit so as to insure the safety of the motoring public.
3. Backfill over 'any portion of the pipeline or utility installation described above that lies within a constructed road-
bed shall be thoroughly tamped to prevent settlement. The top 12 inches of backfill shall be clrushedgravel. Balance of
backfill shall be as stated in special conditions below.
4. Jackson County assumes no liability for and damage which may be caused to the pipeline or utility crossing de-
scribed above due to routine road maintenance or road improvement. It shall be the responsibility of the holder of this
permit to provide for the construction of the pipeline or utility described above in such a manner as to have sufficient depth
and length so as not to interfere with normal road maintenance operations.
5. The holder of this permit shall hold the County of Jackson harmless and blameless from any damages that may be
caused by the installation or maintenance of the above-described pipeline or utility crossing.
6. Special conditions Compliance with the attached "Special Permit Conditions for Underground
Electric Power Line Installation Within County Road Rights-of-Way". Caution signs to be
placed on each side of installation as per the attached sketch. Compliance with the
attached signing detail sheet.
This permit is revocable at any time, and will be strictly adhered to and no work other than that specifically mentioned
above is hereby authorized.
This permit shall be void unless the work herein contemplated shall have been completed before ______n__..__......................

PO~\TER LINE INsrALLATION WITH[N COUNTY ROAD RIGHTS-OF'-WAY
1) Longitudinal power line installations shall not be allowed
within the roadbed section, including ditches, unless the
con.ditions are" such that installation outside the ditch
line would present an extrem~ difficulty. The burden of
proving "extreme difficulty" shall lie with the applicant.
2) When located outside the roadbed section, the cable shall
be placed as near the right-of-way line as possible. while
maintaining a generally uniform distance from the road
centerline.
3) Warning signs shall be placed at frequent intervals over
the cable, including both sides of road crossings.
4) The minimum depth for power cables shall be 36 inches
including roadbed ditch crossings. Under no circumstances
shall the depth of power cable be less than 36 inches unless
contained in steel conduit pipe.
