Enclosed are two copies of the Agreement for Provision of Services of EMT Supervising Physician
for Ashland Fire & Rescue for the 2007-2008 year. This agreement is an addendum to the
agreement for 2006-2007. with changes to two sections: Term and Compensation. Please review
this addendum, sign and return one copy to me.
As you have probably been aware my fees for EMS medical direction will increase significantly for
this contract year. During the past year I have been looking at the cost for me to provide this
service, at how other physicians throughout the state and across the nation are reimbursed, and
at what it would take for Jackson County EMS agencies to replace my services with those of
another physician. As a result I have recalculated my compensation formula (based on your
agency's number of EMS runs and the number of EMTs and First Responders in your agency) to
determine each agency's fee, with a minimum fee of $600/year (or $50/month). I realize that this
increase is substantial and I think that this still represents a great value for Jackson County EMS
agencies.
The total fee for Supervising Physician services by Paul S. Rostykus, MO, MPH for Ashland Fire &
Rescue for the year July 1, 2007 through June 30, 2008 is $6,146.75 due now. This fee can also
be paid on a twice yearly or quarterly basis if that would benefit your agency. Please contact me if
you would like to make such arrangements.

Agreement for Provision of Services
of EMT Supervising Physician
This Amendment to the 2006-2007 Agreement for Provision of Services of EMT
Supervising Physician is made on the date last written below by and between PSR Physician
Services, an Oregon limited liability company ("Physician") and Ashland Fire & Rescue
("Agency") .
The following items are amended to read as follows:
2.0 Term
The term of this Agreement shall be one (1) year, commencing July 1, 2007 and ending
June 30, 2008. Either party may terminate this Agreement at any time for any reason upon
sixty (60) days advance written notice to the other.
5.0 Compensation
Agency shall pay Physician $6,146.75 per year as compensation for the services
performed under this Agreement. Payment shall be made no later than the 30th of September
of the term of this Agreement, unless other arrangements have been mutually agreed to.
