ADDENDUM TO CITY OF ASHLAND
CONTRACT FOR PERSONAL SERVICES LESS THAN $25.000
Addendum made this 8th day of September, 2008, between the City of Ashland (.City")
and Aqu8technex ("Consultant").
Recitals:
A. On August 7, 2008., City and Consultant entered into a "City of AshJand Contract for
Personal Services Less Than $25,000" (further referred to in this addendum as "the
agreement").
B. The parties desire to amend the agreement to -'ncTesse the scope of services and
compensation to be paid Consultant".
City and Consultant agree to amend the agreement in the following manner:
1. The date lor complet/on as specllled In Paragraph 3 01 the agreement /s
extended to September 19, 2008" and the maximum price as specified In
Paragraph 4 of the agreement Is Increased to $65BB).

2. The acope of services Is mod/lied to add a second algaecide application on or
around September 15, 2DOB.
3. Except as modified above the terms of the agreement shall remain in full force and
effect.
