ADDENDUM TO CITY OF ASHLAND
CONTRACT FOR GOODS & SERVICES LESS THAN $25,000
Addendum made this 9th day of October , 2008, between the City of Ashland
("City") and Versatile Concrete. Inc. ("Contractor").
Recitals:
A. On Auaust 28. 2008 , City and Contractor entered into a "City of Ashland
Contract for Goods and Services Less Than $25,000" (further referred to in this
addendum as lithe agreement").
B. The parties desire to amend the agreement to modify the scope of services and
add the additional cost for the modification.
City and Contractor agree to amend the agreement in the following manner:
1. The scope of services is being modified to add the additional height in the columns
that are being constructed and poured at a cost of $800.00. Per attached proposal
dated October 1,2008.
2. Except as modified above the terms of the agreement shall remain in full force and
effect.
