GENERAL PROVISIONS

1, Applicant declares he is the owner or lessee of the real property abutting the above described high-
woy and hos the lawful authority to opply for this permit.
. .-;2" No approach shall be constructed upon the road right-of-way until a signed copy of this permit is
obtoined.
3, This permit is "granted for use only on roads under the jurisdiction of the Jackson County 80a rd
of Commissioners and is not valid for any federal highway, state highway, city or town street.
4. Jackson County ossumes no li,ability for any damage which may be caused to the approoch due to
routine road maintenance or road improvement, It shall be the responsibility of the holder of this permit to
construct the approach to such lines and grades so as not to interfere with normal rood maintenance operations,
5. The holder of this permit shall hold the County of Jackson harmless and blameless from any damages
that may be caused by the installation,

applicant's ownership of the material entering into said 'construction.
7. The Jackson County Department of Public Works is to be notified in writing or by
telephone (826-3122) or (776-7268) 24 hours before starting work.
8. Failure to construct the approach or approaches in conformance with the provisions written herein
or plans attached hereto may result in cancellation of the permit and removal of the partially constructed
approach or approaches at the applicant's sole expense,
9. Proper barricades and warning signs must be maintained at all times during construction by the
holder of this permit so as to insure the safety of the motoring public,

10. The County road is to be restored to its original or to a better condition. All excess rock or dLrt is
to be removed from traveled' portion of the road by brooming, or washing as directed. Final c1eanupfis to
