Under terms and conditions in Blanket Permit No.:22455 the City of
Ashland hereby applies £or permission to install a service connection or service
connections in accordance with the following conditions:

Pipe Size and Kind: 4" Cast Iron

Highway: Rogue Valley Highway H63
Mile Post: 17.98262 and 17.9539.i
Station: 84+65.5 and 83+14
Service Address:
Pipe shall be driven or bored under the surfaced portion of the
highway.
Pipe shall not cross the surfaced portion of the highway.
Open cutting of surface is required.
The work area shall be protected by traffic control devices conforming to and
placed in accordance with the latest provisions of the Manual on Uniform Traffic
Control Devices for Streets and Highways, U.S. Department of Transportation,
Federal Highway Administration.
