II Date of Application: 10-6-81 Effectiae Date: Z, o Foim ]3�315] iAer. 3-19i SP';4F�� RULE The appropriate sections contained in Department of Transpor,tation, Highway Division Administrative Rule �46 Governing ?ole Line, Buried Cable, Pipe Line, Non-Commercial Signs„ and Miscellaneous Operations and/or Facility Permits, february 25, 1931 as supplemented and modified by the following Special Provisions, shall apply to this permit, and by tfiis reference are made a part hereof. It shall be the obligation of the applicant to obtain said Rule from the Department and to determine which of the var.ious provisions are applicable before commence- ment of work under said permit. SPECIAL PROVISIONS 1. The section of highway involved �}s�s/has not= been designated as a Freeway. 2. The section of highway involved c�g/does not° require a permit from the United States Forest Service. 3. _General Provision �L�A regarding insurance shall/x��� apply during the initial installation of facilities. 4. General Provision �B regarding bond sX�shall notT apply during the initial installa- tion of facilities. Bond in the amount of is required. 5. General Provision 1! ,T_ �/shall not' apply to the following crossings of the highway: a. All crossings covered by this permit. b. Crossings as shown on the attached Exhibit Map. If applicable, trenching nearer than feet from the toe of the fill slope or outer edge of surfacing will not be permitted. 6. General Provision �lII,t ��shall not" apply to the following crossings of intersecting roads or streets, road approaches or driveways: a. All such crossings covered by this permit. b. Crossings as shown on the attached Exhibit Map. If applicable, trenching nearer than feet from the toe of the fill slope or outer edges of the surfacing will not be permitted. 7. Open cutting of the paved or surfaced portion of the highway under General Provision i, J" is/�g�t� permitted. 8. The work area during any construction or maintenance performed under the permit provisions shall be protected in accordance with the current "Manual on Uniform Traffic Control Devices for Streets and Highways, U.S: Department of Transportation." 5 If a contractor is to perform this work, prior to commencement of any work, applicant will provide the .contractor an approved copy of this permit and appropriate General Yrovisions. The contractor shall have this material in his possession while work is performed. 10 In consideration of the safety and convenience of the travelling public: Any mud, rock or other debris deposited on the highway surface by any means by the permittee, builder, or supplier(s), shall be immediately removed by sweeping, scraping or flushing with water. Failure to keep the road surface clean shall be considered a violation of the permit provisions, and also ORS 164.785. 11. Applicant, or his contractor, to contact Curt Yocham, Section Supervisor, at 482-4344 at least 3 days prior to commencement of any work, and 24 hours before all work is completed or equipment is removed from the work area.
