ADDENDUM TO CITY OF ASHLAND
CONTRACT FOR :GOODS AND SERVICES LESS THAN $25,000
Addendum made this 24th day of
("City") and Footprint Recyclinq
May, 2011, between the City of Ashland
("Contractor") .
Recitals:
A. On May 10, 2010, City and Contractor entered into a "City of Ashland Contract for
Goods and Services Less than $25,000" (further referred to in this addendum as "the
agreement").
B. The parties desire to amend the agreement to include additional contract provisions.
City and Consultant agree to amend the agreement in the following manner:
1. Contractor to provide an accounting of collections, including pickup dates and
number of gallons collected, with each quarterly payment.
2. Contractor to make quarterly payments to the City of Ashland for the collection of
used cooking oil. Quarterly payments are to be postmarked by March 31st, June
3dh, September 3dh, and December 31st
3. Except as modified above the terms of the agreement shall remain in full force
and effect.
